Medical - Pre-Medical Survey

Introduction

 

  1. The Pre-Medical Assessment (or Survey) is designed to be undertaken by either the subject or in the presence of an operator. The final results would then be available to the Physician / Health Professional. This saves the Physician having to be present during the information gathering phase which can be protracted.

     

  2. The survey can be undertaken in sections over several days if the subject is too busy to complete the entire survey in one 'sitting'. The interface displays both fully completed and partially completed surveys, refer figure 2.

     

  3. The basic start point is shown in figure 1. It provides an explanation of the survey and a list of previous surveys or an appropriate message if no surveys have been taken.

     

  4. A new Survey can be launched using the 'note' icon or the textual link.

  1. The Surveys are listed in chronologically descending order. Partially completed surveys display more information. Refer figure 2.

  1. As the subject completes sections, the remaining section options are displayed. The subject can also elect to finish the survey later at this point. Refer Figure 3.

  1. The subject or operator can send the results of the assessment to any administrator or operator in the same organisation. The Physician or Health Consultant will need to be allocated the security level of either operator or admionistrator. Refer to figure 4 for the interface. This is invoked by clicking on the email icon next to the survey on the start page of the Pre-Med Assessment Tab.

  1. The Assessment results can also be displayed in a modal window by clicking on the  'page' icon alongside the appropriate Assessment.
