Lifestyle - Quick Nutrition Survey

The Quick Nutrition survey is designed to get a snapshot of the Subject's current eating patterns and their understanding of a healthy diet regime.  The section is accessed via the grey tab in the lifestyle review section or via the pull down menu item under the Lifestyle heading.  The information is recorded in an on screen questionnaire with 20 questions that only take a few minutes to answer. 

 

Figure 1 shows the interface for the Quick Nutrition Area. Previous Results are recorded and displayed on the right. The operator can initiate the survey from the 'click here' link.

The Quick Nutrition Survey is a useful tool for screening Subject's and identifying those who have little understanding of a good healthy eating regime and would benefit from a full dietary assessment and healthy eating program.

 

Start the questionnaire by clicking on the 'Start Questionnaire' Button. The Questionnaire is launched in a modal window. (see figure 2)

The first page displays a variety of fields of information which must be completed. These are 'drawn' in from the lifestyle data area, otherwise if the data is missing, it can be completed at this point.  (figure 3).

The questions are typically presented as figure 4 below.

 

More than one answer may be required for example;

 

"does the Subject usually drink coffee with milk or with milk and sugar."

 

All questions need to be answered in turn.

The Subject's Age, Height, Weight and current levels of activity at work and in their leisure time must be recorded in the Lifestyle Review section or on the first screen before completing the questionnaire.  When all of the questions have been answered a report is generated as a pdf report. This is available to review on screen, print or email to the Subject. Refer figure 5.
